Daily rate: Band 8b
Start date: ASAP
Duration: 3 months initially
Location: Midlands
Key criteria:
- Significant experience around understanding delayed discharge and delayed transfer of care (DTOC).
- Experience working with discharge assess models.
- Develop service improvements to discharge planning.
- Support the trust in reducing Length of Stay (LoS).
- Having worked across Trust / CCG / Health and Social Care boundaries.
- A nursing background would be beneficial but not essential.
