Key deliverables / criteria:
- Effectively work with the Continuing Healthcare team and asses the day to day running and ongoing development of the Continuing Healthcare Services.
- Assess, monitor and review complaints procedures of individuals with varying levels of complex needs, whose care is managed in their own homes or in care settings.
- Undertake assessments to facilitate the decision making process for clarification of funding responsibilities and access to Health Funded Step-Up Care.
- Possess expert knowledge and understanding of the process and commissioning approval systems related to Government policies and procedures for Continuing Healthcare provision.
- Provide representation to local Continuing Healthcare Panels and facilitate the Appeals/Review Process for NHS Continuing Healthcare.
- Assess and support teams to ensure that the portfolio of projects are planned, managed and delivered effectively.
